When the forklift operator backs up or places the forklift in reverse in a warehouse aisle where large metal shelving is present to hold product, the shelf face strikes the forklift operator's head and body from behind, crushing the forklift operator in the forklift driver compartment. Unfortunately, these forklift underride accidents are often fatal, or result in brain injury or spinal cord injury.

Typically, forklift manufacturers fail to provide forklift operator protection such as roll bars or anti-intrusion bars.
